Event Registration

NOTE: BY FILLING THIS FORM, YOU ARE AFFIRMING THAT YOU HAVE READ THE IFC RISK MANAGEMENT POLICY AND YOU HAVE EDUCATED YOUR CHAPTER ABOUT RISK MANAGEMENT.

This form is to register parties with the FSILG office as well as the IFC President and Risk Manager. This email will be forwarded to your respective RMCs so they can pass by your event to ensure proper risk management is taking place.

Please fill this form out for a weekend event by the Thursday before the event takes place before 5:00pm. If it is an event taking place during the week, please fill out this form with at least 36 hours notice of the event in question.
